  • When you supply your personal details to this clinic (as part of the medical history) they are stored and processed for one or all of the following 4 reasons (as definied in the Data protection Act 2018, which includes the General Data Protection Regulation, ie the law):

  • • Hold contact details in order to book and manage appointments, provide receipts upon request, and update patients regarding their medical care.

  • • Retain medical records for 8 years following your most recent appointment (or to age 25, if this is longer). After this time, they are shredded securely.

  • Your records are stored

  • • On paper, in locked filing cabinets.
  • • On the office computer (if applicable). This is password-protected, backed up regularly.

  • I will never share your data with anyone who does not need access without your written consent. Only the following people will have routine access to your data:

  • • Myself in order that I can provide you with treatment.
  • • Colleague/receptionist filing the records.

  • I will not share your personal data with anyone outside the Practice without your written consent, unless under legal obligation to comply.
